Every entry from a shift carries the worker, the date and the shift it came from. Print the care notes or any health log for a date range when the auditor asks.

What's Actually in It

1. Care Notes Before They Clock Off

The care note gets written while the shift's still fresh, because the app won't let your worker clock off without one. It lands on the participant's record with the date, the shift and who wrote it, and the next worker can read it before they start.

  • 150 characters minimum, with voice-to-text for anyone who would rather talk it in
  • Group and house shifts: one note for everyone, plus a note for each participant
  • Private notes that only the office ever sees
  • Signature, shift photos, receipts and travel kilometres on the same clock-off form
  • Save the clock-off as a draft on the phone and finish it later
  • Past care notes on the shift in the app, so the next worker knows what happened last time
  • Admins can fix a care note once the shift's completed

2. Nine Health Logs, Written on the Shift

Meds, food and fluids, sleep and hygiene, bowels, diabetes, blood pressure, falls, seizures and weight. Your team logs them from the shift in the app, and each one files itself on the participant, against the shift, with who did it.

  • Medication by the dose: what and how much, the time, and Taken, Refused or Taken Late, with a photo if you want one
  • The participant's medication notes on hand in the app while they log it
  • Readings with their times: blood pressure with its category, up to eight diabetes readings a day with a photo of the meter
  • Bowel movements on the stool chart, from severe constipation to severe diarrhoea
  • Falls with where it happened and any first aid, seizures with the time, how long they lasted and what happened
  • Weigh-ins in kilograms with a photo of the scale, and the latest weight right on the list
  • Every log on the participant's Reports tab, and on the shift it came from

3. Incidents and ABC Reports That Get Followed Up

Something goes wrong, it's reported from the shift while it's fresh, the office hears about it straight away, and the report stays open until someone's investigated it and published it.

  • Incident reports with the type (Level 1, Level 2, near miss or hazard), severity, witnesses and anyone else involved
  • A front and back body map, photos, and whether first aid or emergency services were needed
  • ABC reports for behaviour: what came before, what happened and what came after, with tick boxes for aggression and property damage
  • Admins get an email and an alert in the app the moment one comes in
  • The office records how it was handled, what changes, whether it's notifiable and who to tell, and the investigation, then publishes it
  • Families can lodge an incident or ABC report from their own app too
  • Print it, or download it as a spreadsheet

4. Ready When the Auditor Asks

Everything's dated, has a name on it and sits on the participant it's about, so there's nothing to chase when the audit comes around.

  • Every form a tap away in the shift's menu, on the day

Incident Reports
